Changing Names
If you have recently changed your name or that of your children, or plan to in the very near future it will be necessary to have your passport amended if you plan to travel abroad.
If you are getting married
When getting married there are so many things to organise that changing your name on your passport may well get overlooked. But you should be aware that some countries might not accept an un-amended passport, even if you carry your marriage certificate, so it's well worth having the name changed on your passport well in advance.
We can arrange this for you any time up to three months before the wedding, although you'll be unable to use your passport until the actual day of your marriage.
You will be issued with a new standard 10-year passport. If you are already a passport holder this will mean surrendering your present passport. We will then be issued with a new passport with the change of name. Your new passport will be valid for 10 years and up to a further 9 months depending on the remaining validity on your passport (in whole months, minimum one month, and maximum nine months). Please sign your new name in section 09.
Payment will be as for a new passport. If your present passport has children included they will NOT be re-included on your new passport.
You will need to send the following forms:
- A correctly completed standard application form.
- Form PD2 - completed by the person who will be conducting the ceremony.
Important notes:
- All applications must now include a letter of authorisation to lodge & collect on the clients behalf.
- The form must be current. Code SE/04 should be printed on the top right hand corner of the form. Section 10 must also request the counter signatory's current UK/Irish passport number. To request an original form, email
- The form must be completed in black ink & all signatures must be contained within the boxes.
- Correction fluid must not be used on the form.
- Where requested, daytime phone numbers must be supplied.

All other circumstances
Whatever the reason for changing your name from that on your birth certificate, we will need to see some form of documentary evidence detailing the date and circumstances of that change included with your correctly completed standard application form.
For married or divorced women we will need to see either your marriage or divorce certificate.
If you wish to revert to your maiden name we will need to see your birth certificate, and a statement from you confirming the reversion to your maiden name for all purposes.

Please note that if you have changed your name more than once you are required to submit evidence of ALL changes of name.
