Lost/ Stolen Passports

Requirements:

• Original application form(s)
• Passport photo(s)

Other Requirements:

1. Form & photos must be countersigned (photos must be less than 6 Months old, on a plain/light background and different to photos in previous/ existing passport).

2. One of the following documents must be supplied:

• Original marriage certificate OR
• Original deed poll certificate OR
• Original PD1 & PD2 (forms signed by the clergy/registrar prior to marriage.) OR
• Original decree absolute & original birth certificate (if reverting to maiden name following divorce)

3. All applications must now include a letter of authorisation to lodge & collect on the clients behalf.
